Intraoperative navigation has become an essential tool in modern maxillofacial surgery, particularly for complex anatomical regions. However, its application to mandibular surgery remains challenging due to the mandible's mobility. This report presents a novel approach to resection of a mandibular condyle mass using intraoperative navigation in combination with a Dingman mouth retractor to maintain a stable open-mouth position. A 47-year-old male patient presented with a right condylar osteochondroma. Preoperative computed tomography scans were obtained in maximum mouth opening, and the same position was reproduced intraoperatively using the Dingman retractor. The navigation system was successfully registered and guided precise resection of the lesion. This technique enabled accurate tumor removal while preserving condylar integrity. The case demonstrates that using intraoperative navigation in conjunction with mechanical stabilization of the mandible can improve the accuracy, safety, and feasibility in mandibular surgery.
