Oxygen byproducts induce reactive oxygen species during mitochondrial processes, leading to oxidative stress affecting cellular structures and DNA. Varicocele, a common condition, contributes significantly to male infertility through increased scrotal heat, hormonal imbalances, and testicular hypoxia, intensifying oxidative stress. The consequences include sperm parameter abnormalities and proteomic alterations in seminal plasma proteins. Diagnostic methods like chemiluminescence assess oxidative stress, with sperm DNA fragmentation being a crucial indicator. Management involves surgical interventions, antioxidants, and lifestyle changes, although antioxidant efficacy is debated. This chapter also explores genetic and epigenetic factors in varicocele-induced infertility. Despite promising avenues, antioxidant use requires cautious consideration due to endogenous defense complexities.
